Decision: v2 decisions, open questions and change requests are recorded in this one doc, under the working rules above. No more [Decision] docs.

Why: separate decision docs multiplied and contradicted each other (two were archived on the day they were written), and agents could not tell which one was in force.

Consequences: every session reads this doc first. When the engineering docs move to apps/docs after cutover (D23), this register moves with them and keeps its numbers.
